HOW TO GET A PRESCRIPTION
1- Simply fulfill the medical document (insérer lien) and make your $75 deposit. We will contact you as soon as we have an appointment for you. For any assistance, you may contact us at 819 588-1205 or at email@example.com
2- You will also have to provide a copy of your doctor’s diagnosis of one of the following conditions: osteoarthritis, arthritis, anxiety, depression, insomnia, chronic pain and inflammation, migraines, fibromyalgia, epilepsy, cancer, glaucoma, HIV/AIDS, hepatitis C, anorexia, bulimia, multiple sclerosis, Parkinson’s disease, Crohn’s disease, muscular dystrophy, amyotrophic lateral sclerosis, dystonia and post-traumatic stress syndrome.
*The list of prescription medications in your file at the pharmacy is also eligible.
3- You will have to pay the annual $195 fee, which covers appointment booking, administrative expenses, follow-ups with the nurse, prescription management by a supplier certified by Health Canada and phone support. A non-refundable $75 deposit will be required when you book your appointment. These fees are tax-deductible. If you need to change an appointment, let us know at least 72 hours in advance; otherwise, the deposit will be forfeited.
Payment types accepted: cash, cheque, credit card (Visa and MasterCard).
You’ll receive your prescription for medical cannabis at your first appointment, upon confirmation of your eligibility by the doctor. The prescription will be forwarded directly to one of the suppliers authorized by Health Canada. You will then be able to order directly from this supplier through their website. Products are generally delivered by mail within 48 hours.
WHAT DO I NEED TO BRING WITH ME ?
1. Your health insurance card from the RAMQ.
2. Payment of the clinic fees related to your appointment.
3. Your medical check-up report (summary sheet)
4. A list of the medications on file at your pharmacy or any other report that could help us assess your health (if they have not already been sent to us).
You will have to pay an annual $195 fee, which covers appointment booking, administrative expenses, the initial consultation with the doctor and nurse, the 3-month follow-up appointment, prescription management by a supplier certified by Health Canada and phone support during the treatment. A non-refundable $75 deposit will be required when you book an appointment. These expenses are tax-deductible. Every patient making a request for a medical prescription commits to showing up at their 3-month follow-up appointment. No refunds will be issued for missed or cancelled appointments.
